PHMSA inspection questions test whether an operator identifies fatigue risks, reduces those risks, considers fatigue during incident investigations, designates accountable fatigue management, and establishes an adequate technical basis for controller schedules. The question set also examines alarm management, controller performance, deficiency resolution, records, annual reviews, and compliance with 49 CFR 192.631.
