Gas transmission and gathering incident in BRIDGEPORT, TAYLOR County, WV. Reported cause: EQUIPMENT FAILURE. Reported consequences: 0 fatalities, 0 injuries, 5000 NATURAL GAS released, $0 reported property damage. Operator-reported incident data submitted to PHMSA, generally within 30 days. Cause detail: MALFUNCTION OF CONTROL/RELIEF EQUIPMENT

Cause detail: AT APPROXIMATELY 1:30 AM NOVEMBER 19, 2014 THE EQT CALL CENTER RECEIVED A CALL FROM A MEMBER OF THE PUBLIC ABOUT NOISE AT COMET COMPRESSOR STATION. THE CALL CENTER CONTACTED OPERATIONS PERSONNEL TO RESPOND AND OPERATIONS PERSONNEL ARRIVED AT APPROXIMATELY 2:10 AM TO FIND THE ESD HAD ACTIVATED, AN ESD VALVE HAD NOT CLOSED AND GAS WAS VENTING AT THE STATION SITE. GAS CONTROL ALSO CONTACTED OPERATIONS AND WERE NOTIFIED PERSONNEL WERE IN ROUTE. THE APPLICABLE VALVES WERE SHUT WHICH STOPPED THE VENTING. ONCE CALCULATIONS WERE COMPLETED IT WAS DETERMINED THE GAS LOSS WAS GREATER THAN THE 3 MMCF FOR REPORTING. THE INITIAL ESD WAS DETERMINED TO BE CAUSED BY A BAD GAS DETECTOR SENSOR WHICH WAS REPLACED. IT APPEARS THE VALVE DID NOT CLOSE DUE TO MOISTURE IN THE SUPPLY GAS LINE CAUSING FREEZE OFF.
