Report number: 20140021.

Operator ID: 19160.

Reported incident date: 1/24/2014 20:35.

Location detail: 8-1 + 9.07

Cause detail: DESIGN-, CONSTRUCTION-, INSTALLATION-, OR FABRICATION-RELATED

Cause detail: A LAND OWNER REPORTED HEARING A LOUD NOISE IN THE VICINITY OF TENNESSEE GAS PIPELINE'S (TGP) 100 PIPELINE SYSTEM ON JANUARY 24, 2014 NEAR VICTORIA, TX. TGP PERSONNEL WERE DISPATCHED AND DISCOVERED NATURAL GAS BEING RELEASED FROM TGP'S 24" 100-1 PIPELINE. VALVES WERE CLOSED TO ISOLATE THE SEGMENT WHICH BLEW DOWN TO ATMOSPHERIC PRESSURE. VISUAL EXAMINATION REVEALED ABOUT A 10" LONG X 0.25" CRACK IN THE GIRTH WELD ON THE BOTTOM OF THE PIPE. METALLURGICAL TESTING DETERMINED LACK OF FUSION IN THE GIRTH WELD DUE TO PIPE MISALIGNMENT DURING WELDING TO BE THE CAUSE OF THE CRACK.

PHMSA trend classification: significant incident, not serious.

PHMSA standardized cause: MATERIAL FAILURE OF PIPE OR WELD — CONSTRUCTION, INSTALLATION OR FABRICATION-RELATED.

PHMSA indexed costs: $132,634 reported total cost, $76,117.282 in 1984 dollars, $146,600.904 in current-year dollars.

PHMSA trend flags, standardized causes, and indexed costs are analytical fields added to operator-reported incident data. They do not represent final agency causal findings.
