Dear Mr. Matlovsky:
This is in response to your letter of January 24, 1974, in which
you request statistical data concerning pipeline ruptures in the
western States.
We are enclosing: (1) a copy of Part 191 regarding the leak
reporting requirements; (2) leak reporting forms DOT F 7100.1
and DOT F 7100.2, (applicable for sections 191.9, 191.13, and
191.15); and (3) a summary of 1972 data obtained from forms DOT F
7100.1 and 7100.2 from Region 5 which includes the States of
Alaska, Arizona, California, Hawaii, Idaho, Nevada, Oregon, and
Washington.
We trust that this will provide you with the information that you
are seeking. If we can be of additional assistance, please let
