The item inspected and the\nprobable violation is:\n1. 192.605 Procedural manual for operations, maintenance, and emergencies\nEach operator shall include the following in its operating and maintenance plan:\n(a) General. Each operator shall prepare and follow for each pipeline, a manual of\nwritten procedures for conducting operations and maintenance activities and for\nemergency response. For transmission lines, the manual must also include procedures\nfor handling abnormal operations. This manual must be reviewed and updated by the\noperator at intervals not exceeding 15 months, but at least one each calendar year.\nThis manual must be prepared before operations of a pipeline system commence.\nAppropriate parts of the manual must be kept at locations where operations and\nmaintenance activities are conducted.\nTranscontinental Gas Pipe Line Company, LLC failed to follow its Operations and Maintenance\n(O&M) Manual that requires the review of each policy, procedure and form, the date the reviews\nwere completed, and be documented on an Annual Manual Review Assignment Spreadsheet forms.\nThe reviews are to be completed annually, but not to exceed a 15 month interval.\n\n\n\nCPF 1-2012-1020W\nWGP Safety Training Record showed that this procedure was not followed since pages 17 and 18\ndid not show who reviewed each policy and procedure, and the form was not dated or signed.\nUnder 49 United States Code, § 60122, you are subject to a civil penalty not to exceed $100,000 for\neach violation for each day the violation persists up to a maximum of $1,000,000 for any related\nseries of violations. We have reviewed the circumstances and supporting documents involved in\nthis case, and have decided not to conduct additional enforcement action or penalty assessment\nproceedings at this time. We advise you to correct the item(s) identified in this letter. Failure to do\nso will result in Transcontinental Gas Pipe Line Company, LLC being subject to additional\nenforcement action.\nNo reply to this letter is required. If you choose to reply, in your correspondence please refer to\nCPF 1-2012-1020W. Be advised that all material you submit in response to this enforcement\naction is subject to being made publicly available.
