Patterson\nPresident, Engineering & Project Development\nColumbia Gas Transmission Corp.\n1700 MacCorkle Avenue, SE\nCharleston, WV 25314\nCPF 1-2013-1002\nDear Mr. Patterson:\nOn September 20-23, 2011, State inspectors from the West Virginia Public Service Commission\n(WV PSC), acting as agents for the Pipeline and Hazardous Materials Safety Administration\n(PHMSA), Office of Pipeline Safety (OPS), inspected Columbia Gas Transmission\nCorporation’s (Columbia) Coco Storage and pipeline facilities in Kanawha County, West\nVirginia. As a result of this inspection, Columbia was issued a Notice of Probable Violation and\nProposed Civil Penalty (Notice) on January 24, 2013 in the above-referenced case.\nThis Notice alleged that Columbia was in violation of §192.465(d) by failing to take prompt\nremedial action to correct any deficiencies indicated by the pipe to soil monitoring of its assets\n715769 and 293730.\nColumbia responded to this Notice by letter, dated February 28, 2013. Columbia provided\nadditional information and documentation to show it had complied with the regulation.
