Barger\nVice President, Pipeline Operations\nDominion Transmission, Inc.\n445 West Main Street\nClarksburg, WV 26302-2450\nCPF 1-2013-1014W\nDear Mr. Barger:\nDuring the week of March 5, 2012, a representative of the Pipeline and Hazardous Materials Safety\nAdministration (PHMSA) pursuant to Chapter 601 of 49 United States Code inspected your gas\ntransmission pipelines in the Westfield, PA area.\nAs a result of the inspection, it appears that you have committed probable violations of the Pipeline Safety\nRegulations, Title 49 - Code of Federal Regulations. The items inspected and the probable violation(s)\nare:\n1. 192.605 Procedural manual for operations, maintenance, and emergencies\nEach operator shall include the following in its operating and maintenance plan:\n(a) General. Each operator shall prepare and follow for each pipeline, a manual of\nwritten procedures for conducting operations and maintenance activities and for emergency\nresponse. For transmission lines, the manual must also include procedures for handling\nabnormal operations. This manual must be reviewed and updated by the operator at\nintervals not exceeding 15 months, but at least one each calendar year. This manual must\nbe prepared before operations of a pipeline system commence. Appropriate parts of the\nmanual must be kept at locations where operations and maintenance activities are\nconducted.\nDominion failed to keep appropriate parts of the procedural manual for operations, maintenance, and\nemergencies at locations where operations and maintenance activities are conducted.\nSpecifically, automatic shutdown and manual shutdown procedures were not kept at Harrison Station.\n\n\n\nCPF 1-2013-1014W\n2. 192.605 Procedural manual for operations, maintenance, and emergencies\n(a) General. Each operator shall prepare and follow for each pipeline, a manual of\nwritten procedures for conducting operations and maintenance activities and for emergency\nresponse. For transmission lines, the manual must also include procedures for handling\nabnormal operations. This manual must be reviewed and updated by the operator at\nintervals not exceeding 15 months, but at least one each calendar year. This manual must\nbe prepared before operations of a pipeline system commence. Appropriate parts of the\nmanual must be kept at locations where operations and maintenance activities are\nconducted.\nDominion failed to follow its emergency plan revised 3/1/2006. That emergency plan required\nwritten directions (for field locations), developed either by field personnel or supervision, be\navailable at field offices or support offices.\nSpecifically, written directions were not available at Boom Station.\nUnder 49 United States Code, § 60122, you are subject to a civil penalty not to exceed $200,000 per\nviolation per day the violation persists up to a maximum of $2,000,000 for a related series of violations.\nFor violations occurring prior to January 4, 2012, the maximum penalty may not exceed $100,000 per\nviolation per day, with a maximum penalty not to exceed $1,000,000 for a related series of violations.\nNo reply to this letter is required.
