CPF 2-2012-1005H
Dear Mr. Amundsen:
On July 17, 2013, you wrote to the Pipeline and Hazardous Materials Safety Administration
(PHMSA) Southern Region, Office of Pipeline Safety (OPS) requesting closure of the
Corrective Action Order (CAO) (CPF No. 2-2012-1005H) OPS issued to Florida Gas
Transmission Company (FGT) on December 28, 2012, in response to the December 28, 2012,
incident on FGT’s FLMEA-18 pipeline in Brevard County, Florida.
This letter is to inform you that the OPS Southern Region has reviewed all the actions taken
by FGT with respect to the CAO and we find that FGT has complied with the terms of the
CAO. No further actions are necessary with regards to the CAO. This case is now closed.
Accordingly, all pressure restrictions established by the CAO are withdrawn. FGT can return
this pipeline segment to its maximum allowable operating pressure (MAOP).
Please be advised that this letter refers only to the referenced CAO (CPF 2-2012-1005H) and
not to any other OPS cases.
Sincerely,
Wayne T. Lemoi
Director, Office of Pipeline Safety
