As a result of the incident the Pipeline and\nHazardous Materials Administration (PHMSA) issued a Corrective Action Order (CAO), 4-\n2007-1017H, dated December 19, 2007, requiring Columbia Gulf to take corrective actions with\nrespect to its natural gas pipeline system.\nThe cause of the failure was determined to be external corrosion and Columbia Gulf presented a\nplan of corrective measures designed to prevent additional in-service failures. Through field and\nrecords inspections over the past three years, PHMSA has determined that Columbia Gulf has\nsubstantially completed the requirements of the CAO. Consequently, PHMSA has closed the\nCAO, CPF 4-2007-1017H.\nSincerely,\nR. M.
