The release met the definition of a reportable incident\n\n\n\naccording to §191.3, and a telephonic notification was required as guided in Gulf South\nProcedures (BWP-0009 (Part I), Event Notification Report.)\nThe release occurred due to a failed electrical wiring connection on the valve actuator controls\ncausing a coil to drop out which resulted in the valve opening to the fail safe open position. The\nincident was confirmed by Gulf South at 12:37 a.m. on March 3, 2013, and the incident was not\nreported to the National Response Center until 8:35 a.m., 8 hours after confirmed discovery.\nWe have reviewed the circumstances and supporting documents involved in this case, and have\ndecided not to conduct additional enforcement action or penalty assessment proceedings at this\ntime.\nNo reply to this letter is required. If you choose to reply, in your correspondence please refer to\nCPF 4-2014-1003W.
