KITER\nSeptember 4, 2008\nMr. Perry Richards\nGeneral Manager\nQuestar Gas Management\n1050 17\" St\nDenver, CO 80265\nCPF 5-2008-5027%\nDear Mr Richards\nOn May 20, 2008, a representative of the Pipehne and Hazardous Materials Safety\nAdministration (PHMSA), pursuant to Chapter 601 of 49 United States Code, inspected your\nBlacks Fork Line m Granger, Wyoming\nAs a result of the inspection, it appears that you have committed probable violations of the\nPipehne Safety Regulations, Title 49, Code of Federal Regulations. The items inspected and the\nprobable violations are\n(b) Each operator shall, at intervals not exceeding 7 1/2 months, but at least twice\neach calendar year, inspect each mainhne valve to determine that it is functioning\nproperly.\nThe Main hne valve inspections on the Blacks Forlr. Line were done once a year, not\ntwice per year, not to exceed seven and one half (7-1/2) months for HVL Lines as\nrequired This violation was discovered during the records inspection at the Granger,\nWyommg facihty.\n\n\n\n(a) Except as provided in paragraph (b) of this section, each operator shaB, at\nintervals not exceeding 15 months, but at least once each calendar year, or in the\ncase of pipelines used to carry highly volatBe Bquids, at intervals not to exceed 7/2\nmonths, but at least twice each calendar year, inspect and test each pressui. e\nlimiting device, relief valve, pressure regulator, or other item of pressure control\nequipment to determine that it is functioning properly, is in good mechanical\ncondition, and is adequate from the standpoint of capacity and reBabiBty of\noperation for the sei vice in which it is used.\nThe Overpressure Safety Devices tested on the Blacks Fork Line were done once a year,\nnot twice per year, not to exceed seven and one half (7-1/2) months for HVL Lines as\nrequired. This violation was discovered during the records inspection at the Granger,\nWyoming facihty\nUnder 49 United States Code, ( 60122, you are subject to a civil penalty not to exceed $100, 000\nfor each violation for each day the violation persists up to a maximum of $1, 000, 000 for any\nrelated series of violations. We have reviewed the circumstances and supporting documents\ninvolved in this case, and have decided not to conduct additional enforcement action or penalty\nassessment proceedings at this time We advise you to correct the items identified in this letter\nFailure to do so will result in Questar Gas Management being subject to additional enforcement\nactloil\nNo reply to this letter is required.
