Box 100360\nAnchorage, AK 99510\nCPF 5-2008-5030M\nDear Mr. Isaacson:\nOn July 14-17, 2008, a representative of the Pipeline and Hazardous Materials Safety\nAdministration (PHMSA), pursuant to Chapter 601 of 49 United States Code, inspected your\nOhktok pipeline facihties in Prudhoe Bay, Alaska.\nAs a result of the inspection, it appears that you have committed a probable violation of the\nPipehne Safety Regulations, Title 49, Code of Federal Regulations. The items inspected and the\nprobable violations are:\n(e) Each valve other than a check valve must be equipped with a means for clearly\nindicating the position of the valve (open, closed, etc. ).\n\n\n\nValves 1035 and 1036 located m module 501 were not equipped with easily visible\nposition nidicators. Even after removal of dust covers above the mdicators, the valve\nposition could not be readily discerned\nUnder 49 Uruted States Code, ) 60122, you are subject to a civil penalty not to exceed\n$100, 000 for each violation for each day the violation persists up to a maximum of $1, 000, 000\nfor any related series of violations We have reviewed the circumstances and supporting\ndocuments mvolved in this case, and have decided not to conduct additional enforcement action\nor penalty assessment proceedmgs at this time. We advise you to correct the item(s) identified\nm this letter. Failure to do so will result in Ohktok Pipeline Company being subject to\nadditional enforcement action\nNo reply to this letter is required.
