Specifically, our\nrepresentative inspected the Riverview, Grasslands, and Stateline pipeline assets.\nAs a result of the inspection, it appears that you have committed a probable violation of the\nPipeline Safety Regulations, Title 49, Code of Federal Regulations. The item inspected and the\nprobable violation is:\n1. §195.404 Maps and Records.\n(c) Each operator shall maintain the following records for the periods specified;\n(1) The date, location, and description of each repair made to pipe shall be\nmaintained for the useful life of the pipe.\n(2) The date, location, and description of each repair made to parts of the\npipeline other than pipe shall be maintained for at least 1 year.\n\n\n\n(3) A record of each inspection and test required by this subpart shall be\nmaintained for at least 2 years or until the next inspection or test is performed,\nwhichever is longer.\nThe mid-year inspection record for the only over pressure safety device on the pipeline system\nwas not available for review at the time of the inspection. The missing inspection record was for\nthe calendar year 2013. OneOK switched to a different software system to generate work orders\nand contends that this one inspection was not recorded. Regardless of the maintenance record-\nkeeping system used, OneOK must ensure that over pressure protection devices are inspected at\nthe required intervals and records of the inspection are maintained.\nUnder 49 United States Code, § 60122, you are subject to a civil penalty not to exceed $200,000\nper violation per day the violation persists up to a maximum of $2,000,000 for a related series of\nviolations. For violations occurring prior to January 4, 2012, the maximum penalty may not\nexceed $100,000 per violation per day, with a maximum penalty not to exceed $1,000,000 for a\nrelated series of violations. We have reviewed the circumstances and supporting documents\ninvolved in this case, and have decided not to conduct additional enforcement action or penalty\nassessment proceedings at this time. We advise you to correct the item(s) identified in this letter.\nFailure to do so will result in OneOK being subject to additional enforcement action.\nNo reply to this letter is required. If you choose to reply, in your correspondence please refer to\nCPF 5-2014-5016W. Be advised that all material you submit in response to this enforcement\naction is subject to being made publicly available.
