Background\nA breakout tank exploded and\nsubsequently ignited in Glenpool,\nOklahoma on April 7, 2003. The\naccident involved an 80,000-barrel\nbreakout tank that exploded and burned\nas it was being filled with diesel. The\nresulting fire burned for over 20 hours\nand damaged two other nearby breakout\ntanks. While there were no injuries or\nfatalities, the cost of the accident\nexceeded two million dollars, residents\nadjacent to the accident site were\nevacuated, and area schools were closed\nfor two days.\nThe National Transportation Safety\nBoard (NTSB) conducted an\ninvestigation into the accident and\nsubsequently published a Pipeline\nAccident Report titled ‘‘Storage Tank\nExplosion and Fire in Glenpool,\nOklahoma.’’ In its findings adopted on\nOctober 13, 2004, the NTSB issued a\nrecommendation to OPS to issue an\nadvisory bulletin to liquid pipeline\noperators to validate the accuracy of\ntheir tank strapping tables.\nThe breakout tank that exploded\ncontained an internal floating roof\nsystem equipped with pontoons that\nfloat on top of the product at a certain\nlevel. The tank also had legs that\nsupported the roof whenever the\nproduct was drained and the volume of\nliquid in the tank decreased to the level\nat which the roof no longer floated.\nAdditionally, the tank had two\nSupervisory Control and Data\nAcquisition System (SCADA) alarms to\nalert controllers when the volume was\nnearing the level at which the roof\nwould no longer float. The alarm set\npoints were based on the landed height\nof the floating roof assumed in the\noperator’s strapping table.\nNTSB determined that based on the\nheight measurement of the floating roof\ndocumented on the construction\ninspection report, and based on\nmeasurements investigators made after\nthe accident, the strapping table was\nincorrect. Specifically, the distance from\nthe bottom of the pontoon to the datum\nplate was found to be higher than\nindicated on the pre-accident strapping\ntable. The surface of the charged diesel\nwas within approximately two inches of\nthe pontoons at the time of the\nexplosion. This, according to NTSB, is\nthe most likely time for a static\ndischarge to occur. Based on this\nfinding, as well as other contributing\nfactors, the NTSB determined that an\nincorrect measurement on the strapping\ntable contributed to the cause(s) of the\naccident.\nII. Advisory Bulletin ADB–05–02\nTo: Owners and Operators of All\nPipeline Facilities Who Rely on\nStrapping Tables to Determine Volume\nBased on Measured Height For Product\nLevel.\nSubject: Validation of Strapping\nTables to Reduce the Likelihood of\nErrors That May Lead to Dangerous\nConditions in Breakout Tanks.\nPurpose: To advise owners and\noperators of all hazardous liquid\npipeline facilities about the need to\nvalidate strapping tables.\nAdvisory: Strapping Tables are\ncommonly used to determine the\ncommodity volume based on product\nlevel within breakout tanks. If the\nstrapping table is incorrect, operators\nmay expose themselves and the\ncommunity to unnecessary risks.\nOPS seeks to advise operators that\nthey should review and, if necessary,\nrevise their breakout tank operating\nprocedures to minimize risk. The\nstrapping tables should be validated to\nreduce the potential for errors that may\nlead to dangerous conditions, such as\nstatic discharge inside a tank after a\nfloating roof has been either\nintentionally or unintentionally landed.\nPipeline operators, therefore, may need\nto adjust the measurements on their\nstrapping tables to ensure accuracy.\nIssued in Washington, DC, on March 18,\n2005.\nTheodore L.
