Docket Order: PHMSA-2016-0058Respondent was assessed a civil penalty of $14,200 for seven violations of the HMR when it offered for transportation, and transported, in commerce, a hazardous material (RQ, UN3332, Radioactive material, Type A package, special form, 7), without maintaining complete documentation of tests and an engineering evaluation or comparative data; listed a non-working (un-monitored) emergency response telephone number on the shipping paper; failed to provide emergency response information required for a hazardous material during transportation and failed to have it immediately accessible to facility personnel; failed to maintain records of training for hazmat employees; failed to durably mark with sharply contrasting color the required UN identification number, proper shipping name, "USA DOT 7A Type A" and "RQ" markings on the package; used a shipping paper that failed to list the correct UN identification number, proper shipping name, hazard class, and the activity; and failed to retain a record of each shipment made, to include proper shipping name, identification number, and quantity transported, when using a shipping paper without change for multiple shipments of one or more hazardous materials having the same shipping name and identification number.
