Cause detail: ON 10/29/2015 OPERATION PERSONNEL DISCOVERED A RELEASE AROUND A BURIED VALVE ON THE HUMBLE TO LIBERTY 6" LINE SEGMENT. EXCAVATION AROUND THE VALVE REVEALED A FAILURE OF THE VALVE PACKING. THE VALVE HAS BEEN REMOVED, THE LINE BLINDED AND ASSOCIATED DEAD LEG PIPING REMOVED.
