FAILURE OCCURRED IN BODY SEAL OF VALVE RESULTING IN 25 GALLONS OF CRUDE OIL RELEASED. ANOTHER OPERATOR'S TECHNICIAN NOTICED RELEASED WHEN PASSING BY AND NOTIFIED EOG AT 2:16PM AND CONTROLLER SHUT-IN PIPELINE SYSTEM AT 2:25PM. EOG PIPELINE TECH ARRIVED ON-SITE AT 3:15PM TO CONFIRM AND ISOLATE. EOG'S CONTRACTOR ARRIVED SHORTLY AFTER TO ASSIST IN REPLACING VALVE. PRODUCT STAYED WITHIN PIPELINE RIGHT-OF-WAY AND WAS PROMPTLY BIO-REMEDIATED. FAILED VALVE WAS INVESTIGATED BY EOG, VALVE MANUFACTURER, AND 3RD PARTY SUBJECT MATTER EXPERT (SME). FAILURE WAS CAUSED BY A DETERIORATED O-RING IN BODY SEAL. ALL OTHER VALVE COMPONENTS WERE VERIFIED TO BE DESIGNED AND BUILT TO SPECIFICATIONS. THIS WAS NOT RELATED TO AN OVERPRESSURE EVENT ON THIS SYSTEM. O-RING HAS BEEN SENT OFF TO LABORATORY FOR FULL INVESTIGATION AS TO CAUSE OF FAILURE. LABORATORY RESULTS ARE PENDING AT THE TIME OF THIS REPORT SUBMISSION.","truncated":false,"body_characters":1637}
