Cause detail: PUMP OR PUMP-RELATED EQUIPMENT

Cause detail: ON 04/21/25 AT APPROXIMATELY 05:44, LOCAL ET EMPLOYEE NOTIFIED THE CONTROL CENTER THAT THE LACT SEAL HAD FAILED AND PRODUCT WAS ON THE PUMP PAD. THE CONTROLLER IMMEDIATELY PULLED PERMISSIVE ON 2365 - END WTH 20-8 6INCH GATHERING LINE (INTRASTATE - TAC 8.110(A)(3)). IT WAS DETERMINED THAT THE BEARING ON THE LACT PUMP FAILED CAUSING THE PUMP TO HEAT TO A DEGREE THAT CAUSED THE MECHANICAL SEAL TO FAIL AND CAUSED PRODUCT TO BE RELEASED. APPROXIMATELY 25 BARRELS OF CRUDE OIL WAS RELEASED. CLEAN-UP CONTRACTORS RECOVERED PRODUCT AND REMOVED CONTAMINATED SOIL. THE LEAK WAS REPAIRED BY REPLACING THE SEAL. THE ACCIDENT IS STILL UNDER INVESTIGATION. A SUPPLEMENTAL REPORT WILL BE SUBMITTED ONCE THE ROOT CAUSE FAILURE ANALYSIS IS COMPLETED. SUPPLEMENTAL & FINAL: THE CAUSE OF THE ACCIDENT WAS DUE TO A PUMP-RELATED EQUIPMENT FAILURE. NORMAL WEAR OF THE PUMP BEARINGS CAUSED PUMP TO OVERHEAT LEADING THE SEAL TO FAIL, WHICH ALLOWED PRODUCT TO BE RELEASED. TO PREVENT REOCCURRENCE, A PREVENTIVE MAINTENANCE SCHEDULE FOR THIS PUMP WILL BE ESTABLISHED, AND PROVIDE TRAINING TO PERSONNEL ON PROPER LUBE OIL LEVEL VERIFICATION.
