Cause detail: THREADED CONNECTION/COUPLING FAILURE

Cause detail: ON JULY 14 AT 5:30 CENTRAL TIME, EOG OPERATIONS NOTIFIED THE CONTROL ROOM POC OF A RELEASE AT THE WELL SITE. THE WELL SITE WAS IMMEDIATELY SHUT DOWN AND ISOLATED. THE ON-CALL OPERATOR ARRIVED ONSIGHT AT 6:20 AND ISOLATED THE LEAK. THE RELEASE POINT WAS AT 1" TREADED NIPPLE BELOW THE THERMAL RELIEF VALVE.THE 1" NIPPLE CRACKED IN THE TREADS DUE TO VIBRATION. THE NIPPLE WAS REPLACED WITH A PLUG AFTER THE BROKEN NIPPLE WAS REMOVED AND THE SITE WAS BROUGHT ONLINE.
