Records may be supplemented or corrected and do not represent final agency causal findings.\n\nReport number: 20230092.\n\nOperator ID: 15851.\n\nReported incident date: 10/19/2023 14:01.\n\nCause detail: DEFECTIVE OR LOOSE TUBING OR FITTING\n\nCause detail: ON OCTOBER 19, 2023, AT 2:01 P.M., THE OPERATIONS CONTROL CENTER (OCC) WAS NOTIFIED OF A RELEASE AT THE GUARDIAN PEAK LEASE AUTOMATIC CUSTODY TRANSFER (LACT) UNIT. APPROXIMATELY 1.5 BARRELS OF CRUDE OIL WERE RELEASED. THE CAUSE OF THE RELEASE WAS A 1/2-INCH STAINLESS STEEL TUBING LINE SHEARING AS A RESULT OF SKID VIBRATION THAT EXACERBATED IMPROPERLY INSTALLED TUBING DURING COMMISSIONING. THERE WERE NO INJURIES, AND THE SITE WAS CLEANED UP AND REPAIRED PROMPTLY.","truncated":false,"body_characters":1023}
