Cause detail: THREADED CONNECTION/COUPLING FAILURE

Cause detail: APPROXIMATELY 4 BARRELS OF CRUDE OIL WERE RELEASED FROM A NIPPLE ON A PIT DEVICE CONNECTED TO THE SWEETIE PECK TO CRANE WEST 8" PIPELINE. THE DATE AND TIME OF THE RELEASE (PART A Q4) IS INDICATIVE OF THE EARLIEST POSSIBLE TIME THE RELEASE COULD HAVE STARTED BASED ON CONTROL CENTER INFORMATION. THE CAUSE OF THE RELEASE IS ATTRIBUTED TO EQUIPMENT FAILURE AS A RESULT OF A THREADED CONNECTION/COUPLING FAILURE LIKELY DUE TO VIBRATION. TO PREVENT FUTURE INCIDENTS, THE NUMBER OF FITTINGS WAS REDUCED, LIMITING VIBRATION ON THE PIT DEVICE CONNECTED TO THE PIPELINE.
