THE CAUSE OF THE LEAK WAS IDENTIFIED TO BE A DAMAGED O-RING ON THE CLOSURE. TO REPAIR THE CONNECTION, THE O-RING WAS REPLACED, AND THE CLOSURE WAS PROPERLY SHUT PER MANUFACTURER SPECIFICATIONS. THE CORRECTIVE ACTION TAKEN TO PREVENT RECURRENCE IS TO ADD AN O-RING INSPECTION TO THE MAINTENANCE WORK ORDERS FOR EACH PIG RUN.","truncated":false,"body_characters":1007}
