Records may be supplemented or corrected and do not represent final agency causal findings.\n\nReport number: 20210007.\n\nOperator ID: 30829.\n\nReported incident date: 1/14/2021 11:00.\n\nCause detail: OTHER INCORRECT OPERATION\n\nCause detail: ON 01/14/2021 AT 11:00 AM, A CONTRACTOR WAS AT THE BLUE KRAIT LACT SKID PERFORMING MAINTENANCE ON A VALVE ACTUATOR. UPON REMOVAL OF THE VALVE STEM PACKING GLAND, THE PACKING EXPELLED FROM THE VALVE, CAUSING A SPRAY OF CRUDE OIL. IT WAS DETERMINED THAT THE 3RD PARTY VALVE TECHNICIAN MAY NOT HAVE REFERENCED THE MANUFACTURER'S IOM (INSTALLATION, OPERATION, MAINTENANCE) MANUAL AND MISUNDERSTOOD THE DESIGN OF THE VALVE'S DESIGN BEING WORKED ON. THE IOM CONTAINED A WARNING STATEMENT TO NOT REMOVE THE PACKING GLAND WHILE THE LINE IS UNDER PRESSURE. THE LINE HAD APPROXIMATELY 95 PSIG OF PRESSURE AT THE TIME OF RELEASE. THE LEAK WAS IMMEDIATELY ISOLATED AND CLEANED UP. THE VALVE WAS REPAIRED AND THE SYSTEM WAS RETURNED TO SERVICE. ALL SOIL WAS REMEDIATED. NO FREE LIQUID AVAILABLE FOR RECOVERY.","truncated":false,"body_characters":1341}
