FAILURE INITATED AT MULTIPLE HYDROGEN CRACKS AT THE TOE OF THE FILLET WELD AND AT NUMEROUS ARC BURNS. THE HYDROGEN CRACKING OCCURRED IMMEDIATELY AFTER THE WELD WAS MADE AND WAS CONSISTENT WITH EXCESSIVE WELD COOLING RATES ON STEEL HAVING HIGH CARBON EQUIVALENT.
