AN EMPLOYEE BEGAN OPENING THE 2' BLOW-DOWN, RELEASING GAS TO THE ATMOSPHERE. DURING THE VENTING PROCESS, A SCREWED FITTING SEPARATED, WITH A PART OF THE LOOSE VENT PIPING FATALLY STRIKING THE EMPLOYEE.
