Cause detail: THREADED CONNECTION/COUPLING FAILURE

Cause detail: WTG GAS TRANSMISSION COMPANY WAS IN THE PROCESS OF DE-PRESSURING COMPRESSOR UNIT #7 TO PERFORM MAINTENANCE ON THE UNIT. AN EMPLOYEE BEGAN OPENING THE 2' BLOW-DOWN, RELEASING GAS TO THE ATMOSPHERE. DURING THE VENTING PROCESS, A SCREWED FITTING SEPARATED, WITH A PART OF THE LOOSE VENT PIPING FATALLY STRIKING THE EMPLOYEE. THE VENT VALVE WAS CLOSED SECURING THE RELEASE.
