Cause detail: PUMP OR PUMP-RELATED EQUIPMENT

Cause detail: HILAND CRUDE, LLC (HC) WAS NOTIFIED AT 1:54PM ABOUT A LEAK ON A PRODUCER SITE. WHEN HC OPERATOR ARRIVED ON-SITE APPROXIMATELY BY 2:20PM, HE ISOLATED THE PUMP. SOON THEREAFTER CLEANUP COMMENCED AND WAS COMPLETED THE FOLLOWING DAY. AFTER INVESTIGATION IT WAS DETERMINED THAT A MURPHY HOSE HAD FAILED DUE TO EXCESSIVE WEAR FROM RUBBING AGAINST THE PUMP FRAME DUE TO THE LACK OF PROPER SUPPORT. THE ZIP-TIE THAT WAS INSTALLED TO SECURE THE HOSE HAD BROKEN. FOR THIS EVENT, MORE HOSE SUPPORT WAS NECESSARY APART FROM THE ZIP-TIE ALONE. IT IS BELIEVED THAT DURING THE TIME BETWEEN THE ZIP-TIE BREAKING AND THE HOSE FAILING, THIS CONDITION SHOULD HAVE BEEN NOTICED BY A HC EMPLOYEE. THE HC RICHLAND OPERATING AREA HAS BEGUN INSPECTING 100HP PUMPS IN ITS AREA TO ENSURE PROPER ANCHORING IN SUPPORT OF THE MURPHY HOSES. THE RC 5983 TEAM REVIEWED HOW TO SPOT POTENTIAL ISSUES WHILE ON LOCATION WITH PUMPS. HOSE PROTECTOR SHEATHING WAS ADDED TO THE REPLACED HOSE, THE HOSE WAS REROUTED, AND IT WAS ZIP TIED TO THE PUMP FRAME WITH MORE THAN ONE ZIP-TIE TO PREVENT EXCESSIVE WEARING.
