Cause detail: THREADED CONNECTION/COUPLING FAILURE

Cause detail: ON 3/02/2023, A CHORD ENERGY EMPLOYEE WAS NEARBY AND OBSERVED AIRBORNE MIST. INVESTIGATING, HE DISCOVERED THE CRUDE OIL SPILL AT THE WELL PAD PUMP AND PROCEEDED TO SHUT DOWN THE PUMP. THEN, CONSEQUENTLY, THE CHORD ENERGY EMPLOYEE CONTACTED HILAND CRUDE (HC) STAFF AT 20:44-HOURS. AT 21:05-HOURS, A NEARBY HC EMPLOYEE ARRIVED AT THE CHORD ENERGY RI-LYNN 5201 41-2 5BX WELL SITE, CONFIRMED THE EVENT AND PUMP STATUS, AND THEN ISOLATED THE PIPING AT THE PUMP. OPERATIONS INITIATED THE EMERGENCY RESPONSE LINE (ERL) AT 21:10-HOURS AT WHICH TIME THE CRUDE OIL SPILL WAS DEEMED REPORTABLE. APPROXIMATELY 23.4 GALLONS (0.55 BARREL) OF CRUDE OIL WAS SPILLED WITHIN THE EARTH CONTAINMENT SURROUNDING THE EQUIPMENT. CLEANUP ACTIVITIES WITH CONTRACTOR LABOR AND EQUIPMENT BEGAN AND WAS COMPLETED ON 3/03/2023. THE INTERNAL INVESTIGATION FOUND THE DISCHARGE PIPE FITTING AT THE PUMP HEAD HAD FAILED ALLOWING THE SPILL OF CRUDE OIL. HC REPLACED THE DISCHARGE FITTING AND THE SUCTION PIPING HEADER AND THE FACILITY WAS RETURNED TO SERVICE ON 3/10/2023.
