CONTRACTOR USED A BULLDOZER FOR THE JOB. AS THE BULLDOZER DROVE ON THE BACK OF THE DITCH, A CLEAT ON THE TRACK HIT THE PIPELINE, LOCATED IN THE BOTTOM OF THE DITCH, CAUSING DAMAGE AND THE LEAK. AMOCO PIPELINE DID NOT RECEIVE A NOTIFICATION FROM THE OKIE ONE-CALL SYSTEM REGARDING THIS WORK.
