THE FRACTURE INITIATED NEAR THE OD SURFACE 17 INCHES DOWNSTREAM OF GIRTH WELD 1220 DUE TO HYDROGEN STRESS CRACKING AT A HARD SPOT. THE HARD SPOT WAS A MANUFACTURING DEFECT PRESENT SINCE PLATE ROLLING. THE 3.3 MILES OF PIPELINE (AFFECTED AREA) WAS RETURNED TO SERVICE ON JUNE 29, 2012 AT 80% OF THE OPERATING PRESSURE AT THE TIME OF THE INCIDENT.
