THE APPROXIMATE LOCATION WAS 18736 SWITZER ROAD, IN DEFIANCE COUNTY, NEAR DEFIANCE OHIO. THERE WERE NO INJURIES, LOCAL OPERATIONS CONFIRMED THAT 25 HOMES WERE EVACUATED WITH A TOTAL OF APPROXIMATELY 50 PEOPLE. A ROOT CAUSE FAILURE ANALYSIS WAS PERFORMED TO DETERMINE THE CAUSE.
