Records may be supplemented or corrected and do not represent final agency causal findings.\n\nReport number: 20240036.\n\nOperator ID: 39551.\n\nReported incident date: 1/24/2024 10:13.\n\nCause detail: INTERNAL CORROSION\n\nCause detail: ORIGINAL REPORT WAS SUBMITTED ON 2/23/2024 WITHIN 30 DAYS OF THE INCIDENT; HOWEVER, ACCIDENT INVESTIGATOR BLESSON MATHEW NOTIFIED ON 4/9/2024 IT WAS SUBMITTED ON THE WRONG FORM AND REQUESTED CORRECTED REPORT SUBMITTED WITH NOTE. PINHOLE LEAK ON 8 INCH CRUDE GATHERING PIPELINE. VALVES UPSTREAM AND DOWNSTREAM WERE CLOSED TO ISOLATE LEAK SECTION AND BOLT-ON CLAMP WAS INSTALLED AND LINE WAS RETURNED TO SERVICE. CONTAMINATED SOIL WAS EXCAVATED AND PLACED IN A PLASTIC LINED CONTAINMENT TO BE BLENDED WITH CLEAN SOILS. METALLURGICAL ANALYSIS OF THE FAILED PIPE SECTION DETERMINED THE FAILURE WAS A RESULT OF INTERNAL CORROSION IN TOP OF PIPE. THOROUGH INSPECTIONS UPSTREAM AND DOWNSTREAM OF LEAK LOCATION DID NOT IDENTIFY ANY ADDITIONAL LOCATIONS OF CORROSION PRESENT. AS CORRECTIVE ACTION, STAKEHOLDER IS WORKING WITH THIRD-PARTY INTERNAL CORROSION/CHEMICAL TREATING COMPANY TO EVALUATE CHANGES TO EXISTING INTERNAL CORROSION CONTROL PROGRAM. PROGRAM WILL BE UPDATED AS DETERMINED NECESSARY.","truncated":false,"body_characters":1539}
