AN ENTERPRISE EMPLOYEE DISCOVERED THE INCIDENT AND IDENTIFIED THAT A 2\" BYPASS VALVE AROUND THE ESD ISOLATION VALVE WAS INADVERTENTLY LEFT PARTIALLY OPEN, THUS ALLOWING GAS TO BYPASS THE ISOLATION VALVE AND VENT GAS TO THE ATMOSPHERE. OPERATIONS PERSONNEL CLOSED THE VALVE TO END THE EVENT.
