Cause detail: INCORRECT OPERATION

Cause detail: DURING INSTALLATION OF NEW 16� CONTROL VALVE AT ROSEDALE STATION, FUMES FROM PIPING FLASHED THROUGH A 2" VENT ON TOP OF THE PIPE WHILE WELDING. EARLIER IN THE DAY THE PIPING THAT WAS NEEDING THE WELD WAS SEALED OFF FROM CRUDE OIL VAPORS BY INSTALLING A MUD PLUG ABOUT 12" - 16" FROM THE WELD. ROUGHLY 6 HOURS ELAPSED FROM INSTALLING THE MUD PLUG TO THE FLASH INCIDENT DUE TO FITTING & ALIGNMENT OF THE PIPING AND WELD COMPLETION. THE FLASH TOOK PLACE DURING THE END OF THE LAST OR CAP PASS AND WHILE THE WELDER WAS LOCATED AT THE OPPOSITE POSITION OF THE FLASH. WELDING WAS STOPPED UNTIL NEW PLUG WAS INSTALLED AND ATMOSPHERE WAS MONITORED FOR SAFETY. SEVERAL FACTORS COULD HAVE CONTRIBUTED TO THE FLASH: THE MUD PLUG COULD HAVE BEEN MIXED TOO DRY, INADEQUATE VENTING, TOO MUCH TIME LAPSE, OR PRODUCT MOVEMENT. **SUBMITTED TO SATISFY REQUIREMENTS OF CPF NO. 4-2010-5013 CONSENT AGREEMENT AND ORDER.** UPDATED BY PHMSA ON 11/25/2015 (COMMODITY CLASSIFICATION).
