Cause detail: DURING ROUTINE PATROL A LEAK ON THE PIPELINE WAS IDENTIFIED. THE COORDINATES HAD TO BE RE-VERIFIED PRIOR THE REPORT BEING FILED DUE TO CONFLICTING INFORMATION. 5 509A-1500 PIPELINE SMYS WAS CORRECTED BY OUR PIPELINE ENGINEER, RICHARD SACCO, PE. SUPPLEMENTAL WAS CREATED IN ORDER TO PUT INTO FINAL STATUS AS REQUESTED BY PHMSA ENGINEER.
