Cause detail: MALFUNCTION OF CONTROL/RELIEF EQUIPMENT

Cause detail: ON NOVEMBER 1. 2007 A RELEASE OF CRUDE OIL OCCURRED AS A RESULT OF EQUIPMENT MALFUNCTION AT KIANTONE PIPELINE'S (KPL'S) BOOSTER STATION LOCATED ON BROADWAY ROAD, IN THE TOWN OF PERSIA, NEW YORK. KPL WAS OPERATING AT A LOWER THAN AVERAGE FLOW RATE DUE TO AN ONGOING REFINERY SHUTDOWN. DURING THIS LOW FLOW, PRESSURE ON THE SUCTION SIDE OF THE BOOSTER STATION MAINLINE PUMPS BECAME ELEVATED. THE SAFETY RELIEF VALVE ON THE PUMPS LIFTED AND RELIEVED PRESSURE TO THE SUMP. THE FLOW ALARM ON THE RELIEF LINE FAILED AND DID NOT SIGNAL THE OPERATOR. A KPL OPERATOR IDENTIFIED THE SUMP AS ACTIVE ON THE SCADA SYSTEM (BUT WITH NO ALARMS) AND CONTACTED KPL MANAGEMENT. KPL PRESONNEL WERE ENROUTE TO INVESTIGATE THE RUNNING SUMP WHEN KPL RECEIVED A CALL FROM CATTARAUGUS COUNTY EMERGENCY SERVICES ABOUT A SPILL AT THE BOOSTER STATION. KPL OPERATORS IMMEDIATELY ROUTED THE PIPELINE PAST THE BOOSTER STATION, LOCKED IN ALL STATION COMPONENTS AND SHUTDOWN ALL FLOW AT THE STATION. PRODUCT HAD ALREADY BACKFLOWED OVER THE PUMP BASES AND OVERFLOWED THE SUMP. EMERGENCY CLEANUP SERVICES WERE IMMEDIATELY CONTACTED AND CLEANUP ENSUED. THE RELIEF VALVE FLOW ALARM WAS REPLACED AND THE HIGH HIGH LEVEL SUMP ALARM WAS RESET AND TESTED.
