Cause detail: NON-THREADED CONNECTION FAILURE

Cause detail: AT APPROXIMATELY 11 AM TARGA OPERATIONS WERE NOTIFIED BY A 3RD PARTY PUMPER OF A POTENTIAL CRUDE OIL LEAK AT THE WOLF JONES LOCATION. OPERATIONS ARRIVED ON SITE AT 11:30 AM AND VERIFIED THAT WE HAD A LEAK COMING FROM A BASKET FILTER UPSTREAM OF THE LACT PUMP. THE UNIT WAS SHUT DOWN AND BLOCK VALVES UPSTREAM AND DOWNSTREAM WERE CLOSED TO STOP THE LEAK. ONCE LEAK WAS CONTAINED REMEDIATION EFFORTS BEGAN. THE LEAK WAS DETERMINED TO BE COMING FROM THE LID OF THE BASKET FILTER. THE CAUSE OF THE LEAK WAS DUE A COMBINATION OF TEMPERATURE FLUCTUATION AND EQUIPMENT VIBRATION CAUSING THE SPIRAL WOUND GASKET TO LOSE ITS SEAL. THERE WERE NO MECHANICAL DEFECTS FOUND WITH STRAINER BASKET OR GASKET. ALL EQUIPMENT AND MATERIAL ARE COMPATIBLE TO THE OPERATIONAL ENVIRONMENT IN WHICH THEY ARE SUBJECT TO. TO PREVENT POSSIBLE RECURRENCE THE BASKET STRAINER WAS SERVICED AND GASKET WAS REPLACED WITH NEW GASKET. ALL MAINTENANCE WORK WAS COMPLETE PER MANUFACTURE RECOMMENDATIONS. AS AN ADDITIONAL MITIGATIVE MEASURE, OPERATIONS PERSONNEL HAVE BEEN REFRESHED ON MANUFACTURER MAINTENANCE PROCEDURES.
