KM PERSONNEL AND FIRE DEPARTMENT WERE NOTIFIED BY THE LEASE OPERATOR. THE LEASE OPERATOR SHUT IN THE WELL AND THE POWER. KM ISOLATED THE RESIDUE AND SALES PIPELINES. THE FIRE DEPARTMENT WAS THEN ABLE TO EXTINGUISH THE FIRE. THERE WERE NO INJURIES, ROAD CLOSURES OR EVACUATIONS. THE PIPELINE IS GATHERING TYPE R. THE NATURAL GAS RELEASE WAS 273.3 MCF. ON THE INVESTIGATION OF THE RELEASE AND FIRE, A CRIMP FITTING ON THE FLEX PIPE WAS FOUND TO BE INSTALLED ON THE FLEX PIPE TO A DEPTH OF 1 3/8 INCHES. NORMAL CRIMP FITTINGS ARE INSTALLED TO A DEPTH OF 4.5 INCHES. BECAUSE THE LACK OF DEPTH OF INSTALLATION, THE O-RINGS ON THE CRIMP FITTING WERE NOT COMPLETELY ENGAGED, WHICH OVER TIME, WOULD CAUSE THE CRIMP FITTING TO FAIL. THE FITTING WAS INSTALLED IN 2014. THE ESTIMATES FOR REPAIRING THE DAMAGES FROM THIS INCIDENT WERE RECEIVED ON MAY 21, PUTTING THE REPAIR COST AT APPROXIMATELY $230,000.","truncated":false,"body_characters":1458}
