Cause detail: WRONG EQUIPMENT SPECIFIED OR INSTALLED

Cause detail: ON APRIL 20 AT APPROXIMATELY 13:09, IRONWOOD LOCAL OPERATING PERSONNEL NOTICED A CRUDE LEAK COMING FROM THE LACT UNIT SUCTION STRAINER BASKET. THE OPERATOR SHUT THE SUCTION VALVE TO THE PUMP TO STOP THE LEAK. AND BEGIN THE REMEDIATION PROCESS. IT WAS FOUND THAT THE STRAINER HAD AN O-RING INSTALLED INSTEAD OF A GASKET, AND THIS CAUSED THE RELEASE. THE OPERATOR INSTALLED THE CORRECT GASKET AND PUT THE SYSTEM BACK INTO SERVICE. TO MITIGATE RE-OCCURRENCE OPERATIONS PERSONNEL INSPECTED LIKE EQUIPMENT TO ENSURE THE CORRECT GASKETS WERE INSTALLED.
