Cause detail: THE VALVE WAS ISOLATED AND NOT IN OPERATION AT THE TIME OF THE FAILURE. SUBSEQUENT INVESTIGATION OF THE VALVE INDICATES IMPROPER TORQUEING OF THE MOUNTING BOLTS DURING MANUFACTURING. ALL OTHER VALVES IN THIS PORTION OF THE MANIFOLD AND OF SIMILAR MANUFACTURE WERE INSPECTED FOR SIMILAR DAMAGE. NO ADDITIONAL DAMAGE WAS FOUND.
