Records may be supplemented or corrected and do not represent final agency causal findings.\n\nReport number: 20210033.\n\nOperator ID: 32099.\n\nReported incident date: 3/31/2021 7:45.\n\nCause detail: OTHER EQUIPMENT FAILURE\n\nCause detail: ON MARCH 31, 2021 AT APPROXIMATELY 07:45 COMPANY PERSONNEL DISCOVERED A CRUDE OIL SPILL AT THE CONCHO WAMPA PUMP SITE. AT 07:45 COMPANY PERSONNEL CONFIRMED THAT THE SPILL CAME FROM AN APACHE 3/4 INCH STAINLESS STEEL BALL VALVE ON THE CT203 COG WAMPA 6 INCH NON-REGULATED GATHERING PIPELINE AND SECURED THE SPILL. A THIRD-PARTY COMPANY WAS CALLED TO CLEAN UP ALL FREE PRODUCT AND REMEDIATE THE SOIL. THE CAUSE OF THE SPILL WAS A DEFECTIVE BALL VALVE, THE SAME APACHE BALL VALVE THAT WAS INSTALLED THE DAY PRIOR (MARCH 30). A NEW KJV 3/4 INCH CARBON STEEL BALL VALVE WITH A WELDED SEAL WAS INSTALLED AND AT APPROXIMATELY 10:00 THE LINE WAS BACK IN SERVICE. THE APACHE BALL VALVE THAT LEAKED AND WAS REMOVED THE DAY PRIOR (MARCH 30) AND THE APACHE BALL VALVE THAT LEAKED AND WAS REMOVED TODAY (MARCH 31) WERE BOTH SENT BACK TO THE MANUFACTURER TO DETERMINE THE CAUSE OF THE FAILURES, STILL UNDER INVESTIGATION. TO PREVENT FUTURE SPILLS, A NEW KJV 3/4 INCH CARBON STEEL BALL VALVE WITH A WELDED SEAL WAS INSTALLED TO REPLACE THE FAILED APACHE BALL VALVE AND EMPLOYEES WILL PERFORM ROUTINE VISUAL INSPECTIONS AT THE PUMP SITE.","truncated":false,"body_characters":1666}
