Records may be supplemented or corrected and do not represent final agency causal findings.\n\nReport number: 20210021.\n\nOperator ID: 32619.\n\nReported incident date: 2/20/2021 13:05.\n\nCause detail: VALVE LEFT OR PLACED IN WRONG POSITION, BUT NOT RESULTING IN A TANK, VESSEL, OR SUMP/SEPARATOR OVERFLOW OR FACILITY OVERPRESSURE\n\nCause detail: ON FEBRUARY 20, 2021, HILAND CRUDE, LLC CONFIRMED DISCOVERY OF A 3.2 BARRELS CRUDE OIL SPILL IN WILLIAMS COUNTY, ND AT A WELL PAD SITE OWNED AND OPERATED BY A LOCAL PRODUCER. ALL PRODUCT WAS CONTAINED WITHIN SECONDARY CONTAINMENT AND RECOVERED. NO INJURIES REPORTED. UPON INVESTIGATION, IT WAS DETERMINED THAT THE PUMP-CUP BLEED-VENT, ISOLATION VALVE WAS INADVERTENTLY LEFT OPEN AFTER PERFORMING MAINTENANCE ON THE VALVE, APPROXIMATELY TWO DAYS PRIOR TO THE EVENT, RESULTING IN THE 3.2 BBL SPILL. THE IMMEDIATE CAUSE OF THE RELEASE WAS FAILURE TO CLOSE THE PUMP-CUP BLEED-VENT VALVE ONCE MAINTENANCE WAS COMPLETE. THE ROOT CAUSE OF THE EVENT WAS THE OPERATOR'S LACK OF SITUATIONAL AWARENESS. HILAND CRUDE, LLC HAS INITIATED CORRECTIVE ACTION TO MARK OR TAG THE CORRECT POSITION OF ALL PUMP-CUP BLEED-VENT LINE VALVES AND SHARING THE LESSONS-LEARNED WITH ALL PERSONNEL WORKING AT THE WELL-PAD LOCATIONS TO PREVENT A REOCCURRENCE OF THIS TYPE OF EVENT.","truncated":false,"body_characters":1607}
