THE HANDWHEEL ON THE VALVE WAS BROKEN AS WELL AS A VALVE GREASE PLUG. THE C3 MIX WAS RELEASED AT A VERY SLOW RATE FROM THE BROKEN GREASE PLUG. A NEW VALVE HAS BEEN INSTALLED AND IS OPERATIONAL. THE ACCIDENT WAS CAUSED BY A NATURAL FORCE (HURRICANE).
