Records may be supplemented or corrected and do not represent final agency causal findings.\n\nReport number: 20230065.\n\nOperator ID: 15851.\n\nReported incident date: 7/4/2023 6:45.\n\nCause detail: DEFECTIVE OR LOOSE TUBING OR FITTING\n\nCause detail: ON JULY 4, 2023, AT 6:45 A.M., THE OPERATIONS CONTROL CENTER (OCC) RECEIVED A HIGH DISCHARGE ALARM AT THE JASMINE LEASE AUTOMATIC CUSTODY TRANSFER (LACT) SITE. THE OCC IMMEDIATELY SHUT DOWN THE LACT AND DISPATCHED OPERATING PERSONNEL TO INVESTIGATE THE CAUSE OF THE ALARM. OPERATING PERSONNEL ARRIVED ON-SITE AND OBSERVED A RELEASE OF APPROXIMATELY THREE (3) BARRELS OF CRUDE OIL. THE RELEASE ORIGINATED FROM A SHEARED 3/8-INCH TUBING LINE ON THE PILOT ASSEMBLY OF THE BACKPRESSURE CONTROL VALVE. THERE WERE NO INJURIES, AND THE SITE WAS CLEANED UP AND REPAIRED PROMPTLY.","truncated":false,"body_characters":1127}
