Cause detail: ON MARCH 12, MARATHON PIPE LINE (MPL) PERSONNEL DISCOVERED DIESEL SPRAYING FROM A VENT HOLE ON TOP OF A 1/2 INCH CONSOLIDATED THERMAL PRESSURE RELIEF VALVE AT MPL'S LIMA METERING PIPELINE STATION. THE SYSTEM WAS PROMPTLY SHUTDOWN AND THE RELEASE WAS CLEANED UP BY EXCAVATING THE CONTAMINATED SOIL. THE TOTAL RELEASE VOLUME WAS ESTIMATED AT 75 GALLONS AND ALL RELEASED PRODUCT WAS CONTAINED ON SITE. THE VALVE WAS REPLACED AND THE LINE RESTARTED. THE VALVE O-RING WAS FOUND TO BE SWELLED AND ROLLED UP OUT OF POSITION. FAILURE ANALYSIS DETERMINED THAT THE MANUFACTURER SUPPLIED INCOMPATIBLE O-RING MATERIAL THAT DID NOT MEET OPERATOR SPECIFICATIONS. 9/15/17 UPDATE: PART D, QUESTION 8: FINAL COSTS WERE UPDATED. PART H: NARRATIVE WAS UPDATED.
