120113002M_Closure Letter_10142011_text.pdf, page 1Official PDFU.S. Department 820 Bear Tavern Road, Suite 103 Of Transportation West Trenton, NJ 08628 Pipeline and 609.989.2171 Hazardous Materials Safety Administration Overnight Express Mail October 14, 2011 Jeffrey Barger Vice President Pipeline Operations Dominion Transmission, Inc. 445 West Main Street Clarksburg, WV 26301 CPF 1-2011-3002M Dear Mr. Barger: During the week of November 15, 2010, a representative of the Pipeline and Hazardous Materials Safety Administration (PHMSA) pursuant to Chapter 601 of 49 United States Code conducted an on-site pipeline safety inspection of Dominion Cove Point LNG, L.P.’s (DCPLNG) procedures and plans at the DCPLNG Plant in Lusby, Maryland. As a result of the inspection, a Notice of Amendment (NOA) was issued on May 5, 2011, which proposed an amendment to DCPLNG’s written plan for personnel health. DCPLNG responded by a letter dated June 3, 2011, requesting a time extension which PHMSA granted. On July 13, 2011, DCPLNG submitted its personnel physical form, Cove Point LNG Physical/Functional Safety Sensitive Job Requirement, in response to the NOA. Having reviewed the aforementioned form, PHMSA provided DCPLNG additional time to incorporate the form into its written plan. On September 13, 2011, DCPLNG submitted its written plan, Section 110, SOP 01 and revised personnel physical form, Cove Point LNG Physical/Functional Safety Sensitive Job Requirement CP0022A. Please be advised, we have reviewed the above documents and it appears that the inadequacies indentified in the NOA have been corrected. Therefore, this letter is to inform DCPLNG no further action is necessary and this case is now closed. Thank you for your cooperation.
