Sheppard Vice President, Pipeline Operations Dominion Transmission, Inc. 445 West Main Street Clarksburg, WV 26301 CPF 1-2015-1007M Dear Mr. Sheppard: From March 11 to April 5, 2013, representatives from the Pipeline and Hazardous Materials Safety Administration (PHMSA), Office of Pipeline Safety (OPS), pursuant to chapter 601 of 49 United States Code, inspected the Dominion Transmission, Inc. (DTI) Operations and Maintenance procedures in Clarksburg, West Virginia. As a result of the inspection, DTI was issued a Notice of Amendment on April 15, 2015, which proposed amendment of your procedures. DTI submitted its amended procedures on May 15, 2015. My staff reviewed the amended procedures, and it appears that the inadequacies outlined in this Notice of Amendment have been corrected. This letter is to inform you no further action is necessary and this case is now closed. Thank you for your cooperation.
