(MarkWest) Maytown to Ranger pipeline in Floyd County, Kentucky on November 8, 2004, and pursuant to Section 60112 of Title 49, United States Code, the Pipeline and Hazardous Materials Safety Administration (PHMSA) issued a Corrective Action Order (CAO) on November 18, 2004, to MarkWest. The CAO was amended on November 19, 2004, and on March 21, 2005. The CAO required MarkWest to take certain corrective actions to protect the public and the environment. Subsequent to the issuance of the CAO and amendments MarkWest Hydrocarbon, Inc. changed its name to MarkWest Energy Appalachia, LLC. This letter is to inform you that MarkWest has complied with the terms of the CAO. No further action is necessary with regards to the CAO and this case is now closed. Please be advised that there is at least one other PHMSA case pending with regards to the above referenced pipeline failure and that this letter refers only to the CAO (re: CPF 2-2004-5017H) and not to any other PHMSA pending cases.
