As a result of the inspection, Mid-Valley was issued a Notice of Amendment on November 23, 2010, which proposed amendment of your procedures. On April 30, 2011, Mid-Valley submitted its proposed revisions. Additional procedure revisions were subsequently submitted on February 20, 2012, and May 29, 2012. My staff reviewed the amended procedures and it appears that the inadequacies outlined in the Notice of Amendment have been corrected.
