As a result of the incident the Pipeline and Hazardous Materials Administration (PHMSA) issued a Corrective Action Order (CAO), 4- 2007-1017H, dated December 19, 2007, requiring Columbia Gulf to take corrective actions with respect to its natural gas pipeline system. The cause of the failure was determined to be external corrosion and Columbia Gulf presented a plan of corrective measures designed to prevent additional in-service failures. Through field and records inspections over the past three years, PHMSA has determined that Columbia Gulf has substantially completed the requirements of the CAO.
